As of July 1, 2011, the Visa and Passport Office of the Chinese Consulate General in Houston will require all applicants to submit the 2011 edition of the "Visa Application Form of the People's Republic of China". The Visa and Passport Office would like to bring applicants' attention to the following:
1.As of July 1, 2011, all applicants are required to apply with the 2011 Edition the "Visa Application Form of the People's Republic of China" (Form v.2011A)(click to download). For work visa \study visa applicants, or if someone else travelling with you shares the same passport with you, or if you are applying for a visa in a country or territory other than the country of your nationality, a supplementary "Visa Application Form of the People's Republic of China" (Form V.2011B) is required.
2.Please follow the instructions(Form V.2011A) and instructions(Form V.2011B) in completing the Visa Application Form.
3.Previous editions such as Form Q-2007 or forms downloaded from other sources will not be accepted.
4.The Application Form must be fully completed on a computer and clearly printed out except that the signature should be hand written. Altered forms or hand written forms will not be accepted.
5.No hard copy of the Application Form will be provided in the Visa Hall. Please make sure that all paper work is in order before you submit your application.
